My brother was suicidal and hospitalized for depression,anxiety, alcholism. He has been in for a month and the doctor's are sayng the meds are not working and are recomending shock treatments. Both he and his wife do not agree with this. The hosp said the insurance will not continue to pay unless they see documentation of improvements. She sees progress and feels they are not doing everything they can to help him. Ask for a second opinion. If this is employer paid group insurance, does brother also have an Employee Assistance Program (EAP)? They can be of great assistance. Also, the hospital generally has a patient advocate - ask to speak with them. Finally, the managed care coordinator at the insurance company can also be contacted. There are dual-diagnosis mental health programs available (mental illness combined with alcohol/substance abuse). They range from inpatient to programs designed for working people. They combine medication management, counseling services, and substance abuse treatment with required attendance at NA/AA meetings.
The cost of inpatient mental health treatment is exorbitant - room and board alone can be $1000/day. The insurance company wants him discharged. The hospital cannot discharge him as long as he is a threat to himself or others. That doesn't mean he is getting the right kind of care needed.
My suggestions are based upon personal experience and nothing else.

Call health insurance company or the provider for mental health services (many insurance companies contract this piece out to a mental health managed care company) and ask about the process for a second opinion.
If you don't already have an authorization signed by him, you will need one to talk to anyone about his care. Insurance company can provide this.
