I'm not sure if you're asking about medical mileage or work related mileage but I'll do my best to answer.

First of all, medical mileage is deductible but the rate is pretty low. You can take mileage for trips to doctors, hospitals, treatment centers, pharmacies, etc. You just need to have a written record/log book of the mileage.

Gas/mileage for WORK purposes are different. If he is traveling from his home to his MAIN place of employment....it is NOT deductible (this is considered to be his normal commute). If it is a TEMPORARY location that he has been assigned to go to (lasts a year or less), then he can take the mileage, but has to keep written logs/books. If this is travel BETWEEN two jobs...he can take the mileage from job #1 to job #2, but not the mileage from or back to home.
