I've had magic jack for a couple of months. It cost $40 to buy (which includes your first year of service) and then it will cost $20 per year to continue service. No other fees. You have to have a high speed connection to use it because the phone service goes through your computer. I have cable; I'm not sure if/how it works if you have DSL. You can make all the domestic calls you want. I don't know what you have to do for international calls.
It was pretty easy to set up. There's only a few steps involved and the only computer knowledge you need is to know how/where to plug it in (you just use a usb port on the computer).
I have had so-so service with it. Sometimes when I make a call, there is an echo on the phone, and such a time lag from the other end that it is almost unusable. From what I've read, the reason it does this sometimes is because of the phone carrier whoever you're calling has. Other times, it works perfectly.
I do have a really lousy phone that I've always had problems with, so that may contribute to my problems (although I've read of other people have similar problems with that echo on the phone).
The other big downfall of it is that if your computer goes, you're out of luck with your phone service. Or if the electricity goes out, same thing.

theres a few it wont work on.........so u might have to ask.......my brothers cuts in and out also..............
