i have a notebook that i keep a list of the forms i have sent in for,this is
especially great for those long exp date forms,this is the way i have it setup
# of forms i have--name of form--box#--exp date--amt to rec
edited to add--i update my list often and the # of forms i
have is on the left side of the red/blue line on the page,this way i can
see the name of the form easily :O)
