Says you can Scan the QR code or send in receipt with name address to Inmar, Laredo, Tx. Certificate must accompany your request and can do 5 per address and 5 in same envelop.

Pls share with me your experience and thoughts on this question----
Do these new trinchero forms that have no lines on them for filling in your naz, actually pay if you attach a 3 x 5 card with naz and add yr crt? I have heard this in the past but I believe it was based on past experiences from maybe 2-3 yrs ago. Have YOU or has anyone known to you been paid on the ones that started 4/1/23 by using this mail in method? I have not been able to get a clear, definitive answer from anyone I have asked.... I CAN tell you this--- the Trinchero brands that used to say you may submit 3 or 5 per env used to pay on all you submitted per env but NO LONGER do. I am hoping this will save many of you wasted time and form costs. I recently called to ask why I am getting post cards that say invalid--you already used this crt on a previous offer when , in fact, the form says you may send 3 or 5 or whatever in one env. The person I spoke to said their computer cannot recognize m multiple submittals in one env and is programmed to reject them.... She was aware that this contradicts the writing on the certificate and had no further explanation. Basically, they will NOT pay on more than one and there is no point in mailing several. You are wasting your time and money. I hope this is helpful info to others like it was to me. I have not yet tried any of the Trincheros that start 4/1 by mail in method so am wondering whether anyone HAS mailed in and been paid. I DID try the LUX24AJ23 $24 online at the site given on the form-- submitted it online in mid May and the site says Invalid or Ineligible Submission-- with no reason given. This is definitely food for thought and I hope to get more info to confirm or dispute my suspicions about their willingness to pay. Hoping for good news/prepared for any news.

I was told, just like salsafan mentioned previously, that their system doesn't recognize multiple submissions in the same envelope and it is programmed to reject them.
